Admission Process
Knowing how to get admitted to Beaumont ahead of time will make things easier when a family member needs short- or long-term rehabilitation or skilled nursing care.
Long-Term Care
Prospective long-term care residents and their family members should tour the facility and complete an application as soon as possible.

Admissions Procedures Admission to Beaumont, whether from the Hospital or from the Community, generally takes place in five easy steps.

Step 1. APPLICATION

Admission from Hospital
Step 2. REVIEW
The Beaumont Admissions Coordinator will talk with the Case Manager or Social Worker to obtain patient referral information. A Liaison Nurse will conduct an on-site evaluation of the patient within 24 hours of the patient’s anticipated discharge date. Based on the information gathered in the medical evaluation, the patient will be considered by the Admissions Committee for admission to one of Beaumont’s post-acute rehabilitation or long-term care programs.

Step 3. NOTIFICATION
The Admissions Committee will make a determination, based on medically appropriate availability, and will communicate a decision within one business day of the review.

Step 4. SCHEDULE ADMISSION
Call the Admissions Department to arrange the date and time of patient admission. If more than 72 hours has elapsed between the time of evaluation and the planned admission, updated medical records may be necessary.

Step 5. ADMISSION
Medical records (including the referral from the Case Manager or Social Worker, discharge summary, and the original comfort care form) must accompany the patient on the day of admission. Health Care Proxy and Power of Attorney forms, along with Medicare and/or other insurance cards also should be presented to the Admissions Coordinator at this time. We recommend a family member accompany the patient throughout the Admissions Process if possible.
